Hospital pharmacist job description:

The outlook for hospital pharmacists remains strong and remains an essential industry for employing pharmacists. Pharmacists can work in an in-patient or out-patient setting, can be clinical or staff in nature, and can also work in management. Many pharmacists chose to work in a hospital setting because of the direct patient contact and use of their specialist knowledge. They have to work collaboratively with other healthcare providers and are often consulted for their expert opinion on devising the most appropriate treatment for patients.

As a staff pharmacist or clinical pharmacist you must show demonstrated skills in collaboration, teamwork and problem-solving, computer skills with pharmacy systems such as Star Pharmacy, Micromedex, knowledgeable with pharmacy dosing protocols, pharmacokinetic dosing.

As a pharmacist in a hospital environment you won't just fill prescriptions, you will build lasting relationships with patients. In this key role you will review and interpret medication orders to identify inappropriate doses, therapeutic and chemical incompatibilities, potential allergic reactions, therapeutic duplications and determine that the therapy is consistent with the patient’s treatment goals.

Resolve communication and clinical issues with the prescriber or responsible physician.

Follow departmental policies and procedures in accordance with state and federal laws in preparing and dispensing medications.

Use pharmacy and hospital computer systems to process medication orders, generate reports and monitor patient drug therapy.

Participate in clinical monitoring, D.U.E. and dosing service as needed; participate in studies/research.

Participate in orientation, training and education of employees and provide supervision for pharmacy technician activities.

Provide product and clinical information requested by hospital’s medical and clinical staffs.

Discuss professional and operational problems with Pharmacy Director.

As a pharmacist you might be responsible for providing pharmaceutical care services to the patients of the hospital including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems.

Experienced pharmacists may be involved in teaching clinical staff on various aspects of drug treatment and efficacy.

Compounding and dispensing medications.

Makes the appropriate IV to PO conversions of selected drug classes.

Prepares IV solutions, or supervises the technicians and interns in the preparation process.

Monitors TPN lab values with the assistance of the clinical pharmacist.

LTC pharmacist job description

Consultant pharmacy jobs are found in the long term care industry and offer flexibility, autonomy, and a require a combination of clinical expertise and business acumen. A consultant pharmacist is responsible for overseeing the long term care facility or skilled nursing facility (SNF) by performing monthly drug regimen reviews and developing programs and policies that meet all state and federal laws.

As a Pharmacy Consultant you are responsible for accurate drug information, noted as an expert resource for nursing and physicians, primary goal is to reduce healthcare costs and increase the overall quality of life for the patient, perform monthly drug regimen reviews, generate educational programs, improve quality control, participate and plan pharmacy committee meetings, develop and implement all activities related to medicaid drug program, researches program policies, develops and interprets federal regulations and federal and state laws to ensure accuracy or program policies and procedures, reviews and recommends institutional pharmaceutical practices.

Supervises the institutional pharmacy through the performance of such tasks as ordering regular pharmaceutical supplies; preparing needed compounded drugs, packaging all unit dose medications, dispensing all unit dose medications, and reviewing drug inventories.

Writes administrative rules, provider manuals, and plan materials, which is correct and consistent with state and federal regulations, laws and policies; responds to requests for information, requests for policy interpretation, requests for exception to policy and requests for budget projections and participates in special studies and special projects related to areas of responsibility and appeals or exceptions to policy.

Reviews and consults with the Division Administrator on the annual development of the budget for the Medicaid drug program; determines financial and technological resources, evaluations, reporting procedures and impact upon program recipients.

Directs and administers the work of assigned professional and technical personnel engaged in specialized medical service, health benefit plan, and technical programs, as necessary.

Oversees the functions and activities of the Medicaid Drug Utilization Commission; manages implementation of Medicaid policy related to the program, the fiscal agent and providers.

Develops policy and monitors fiscal agent operation of drug prior authorization and drug rebate programs.

Represents the Department in public relations regarding pharmacy and drug matters; acts as liaison officer between the Division of Medical Services and other state agencies, groups, boards, consumers and the Medical Assistance Advisory council on pharmacy and medical science matters.

Recommends the acquisition or deletion of pharmaceutical equipment; provides materials management services for designated institutions by acting as wholesaler or distributor of pharmaceuticals and health related items.

Provides consultation to the Division Administrator on such matters as proper drug therapy and in-service pharmaceutical training.

Reviews drug histories and patient profiles of residents to insure proper and safe drug therapy.

Participates in development of management for health care services within the departments.

Analyzes and evaluates program implementation and effectiveness.
